2.The application report of the enterprise;

3.The explanation of the enterprise on the means of supervision control & the guarantee letter of the enterprise for Non-usage of poison-making & non-reselling its products;

4. The original copy of effective import contract signed with foreign customers;

5. The ratified certificate by the local foreign economy & trade bureau for the set-up of the enterprise, the ratified certificate of foreign entrepreneur investment enterprise, enterprise JV contract & its chapters, the business license certificate of foreign entrepreneur investment enterprise & its capital-inspecting report;

6. The supervision control views issued by the local responsible customs;

8. The import enterprise must, down-to-earth, fill in (The import application form for easy-made poisoNous chemical products) according to the requirement of the contents;
